Optimised Healthcare of the Elderly in the Emergency Department/ the Acute Inpatient Treatment  – An Implementation-Study to Evaluate the “Geriatric Check”

The demographic change causes a need for an altered healthcare system. Frequently the treatment of older patients is marked by chronical illnesses and functional limitations. This leads to special requirements to the medical, nursing and psychosocial structure of supply, which constitutes a challenge for the collaboration of health care and social systems. Without the application of a systematic assessment cognitive, depressive or functional impairments, as well as compensatory mechanisms can be easily overlooked. The “Geriatric Concept of Baden-Württemberg” claims to quickly identify and stratify the risks imposed to elder patients in regards to functional and cognitive impairments within the emergency department and in the acute inpatient setting. Therefore geriatric screening and assessment should become an integral part of the treatment of elder patients. The aim of the dissertation project is to validate the “Geriatrics-Check” which will be compared with previous versions of geriatric screening and assessments. An evaluation of the interface is planned with regard to the further treatment of geriatric patients. The purpose is to reduce renewed readmissions in the emergency department, rehospitalisation, admission to nursing home and total mortality.
