Life story work in residential long term care of persons with dementia – Care staffs‘ subjective theories

Any person in need of care, claims his or her individual needs, preferences and aversions to be regarded. Consideration of this individuality is a crucial criterion for good nursing and caring. Professional carers also agree with this. In order to meet this demand, they need comprehensive information about the life of the person in need of care, about meaningful events, values, preferences, aversions, etc., but also about what is currently important for her or him. Life story work means to assess this information systematically, to record it, to communicate it to the team, and to integrate it into care practice. Because of the importance of life story work, professional carers in residential long term care institutions are required by law to provide life story work. 

In care of people with dementia life story work is of special importance: Situations that were meaningful in the past for them often intermingle with the current situation. Only knowledge about the biographical background will allow to understand the experience and behavior of the person with dementia. And only knowledge and intelligent, sensitive use of meaningful “anchors” (e.g. hobbies, professional backgrounds, important persons etc.) will make it possible to build a personal relationship with the person in need of care and to make situations emotionally positive for her or him.

However, there is some evidence referring to discrepancies between life story work as routinely performed in residential long term care practice and the principles recommended in scientific literature. The underlying reasons are not explored properly yet. This PhD project wants to contribute to closing this research gap. As professional carers are the persons doing life story work and knowing the framework conditions in residential long term care by their own experience, this project focuses on their expertise.

The following steps for collecting data are planned in different nursing homes:

• Interviews with leaders and professional carers
• Group discussions with professional carers

Furthermore it may be necessary to do participating observations in public rooms and analyses of the nursing documentation for getting an insight in organizational matters. These data sources are to provide information about professional carers’ experiences in doing life story work with persons with dementia, how they define life story work and what they include in it, which conditions in their opinion facilitate or hinder doing life story work, and when life story work is regarded as successful or unsuccessful.

The aim of the study is generating a theoretical framework in order to explain how life story work with persons with dementia in residential long term care is performed and how it can be improved. These findings will be the basis for the development of a concept. This concept aims to help analyzing and optimizing the performance of doing life story work with persons with dementia.
